Cochranton Junior-Senior High School

Complete this application if you are a graduating senior attending Cochranton Junior-Senior High School. Please review the scholarship instructions and criteria below before submitting your application.

APPLICATION
Complete the on-line application form. Applications will be accepted through 5:00 PM on April 1. All supporting documents can be uploaded in the scholarship application system. If items are to be sent via US Mail, they must be postmarked or delivered to the Crawford Heritage Community Foundation, PO Box 933, Meadville, PA 16335- 0933, no later than April 1. Incomplete applications will not be considered.

SECONDARY SCHOOL TRANSCRIPT
The applicant is responsible for having their official secondary school transcript, including courses taken, rank in class (if applicable) and ACT and/or SAT® scores (optional), submitted to the Foundation. The transcript should include mid-senior year grades.  Transcripts can be uploaded in the scholarship application system, or emailed to philanthropic@crawfordheritage.org

ENTRANCE EXAMINATIONS (Optional)
Attach a copy of your ACT and/or SAT® score notification from The College Board if your scores are not listed on your secondary school transcript.

FINANCIAL AID APPLICATION
Attach to your application a copy of your Federal Student Aid Report (SAR)*, your most recent IRS Form 1040 and one for each parent. If either parent fails to supply an IRS Form 1040 you must include a statement indicating your effort to obtain the form and the contribution toward your education expected from that parent.

Please mark out ALL social security numbers listed on submitted forms to help ensure the security of your and your parents’ identities! DO NOT submit attachments to IRS Form 1040—this includes Forms W-2 and 8863 and Schedules A, B, C, D and SE.

NOTIFICATION
Scholarship recipients are notified of their awards in early June. Students not selected to receive scholarships will not be notified due to the volume of applications received. You may visit the Foundation web site at www.crawfordheritage.org to review a complete list of scholarship awards.

* You must complete the Free Application for Federal Student Aid (FAFSA) form (www.fafsa.ed.gov) to receive your Federal Student Aid Report (SAR). This form can be completed as soon as January 1st of the year you plan to attend college. You can go back to your FAFSA to check status, make corrections to a processed FAFSA and get other information should your circumstances change. Four to six weeks after submitting your FAFSA, you will receive your Student Aid Report (SAR) which
summarizes the information you reported. (A sample SAR is available for your reference on our website at www.crawfordheritage.org/docs/CompletedSARsample.pdf.) Applications missing your SAR or other supporting documents will not be considered so please plan ahead! Contact your guidance counselor for more FAFSA information.
